VSA Nevada at the Lake Mansion
BIRTHDAY PARTY RENTAL AGREEMENT 
Cost:  $200 for up to 12 children due at time of reservation - $10 per additional child - 20 children max due on day of event.
 
Includes:   Use of Art Space in the lower level of the mansion for 2-1/2 hours (up to 2 hours for the party plus set-up & clean-up), an age appropriate 45-60 minute creative activity (art, creative movement OR acting) conducted by one of our experienced art teachers for up to 20 children, a $20 gift certificate for the birthday girl or boy to use toward a future VSA Nevada workshop or camp (may not be used toward party rental), tables, chairs, and all needed art supplies for a creative project that each child will be able to take home.                    

Hours: Parties may be scheduled Monday through Sunday from 9am-7pm, upon availability. To make your reservation and payment, VSA Nevada Office hours are Mon-Fri 9am-5pm.  ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
FACILITY USE RULES  
 - User is responsible for any injury that might occur on the grounds or in the building.
 - User is responsible for set-up and clean-up. 
 - No furnishings or other materials should be moved without prior approval.
 - Children must be under adult supervision at all times.   
 - No animals are allowed in the mansion or on the grounds except for guide animals.
 - ABSOLUTELY NO SMOKING is allowed in the mansion, on the porches and grounds.
 - A refrigerator is available for use during your party. Anything left after your event will be discarded.
 - No birdseed, rice, confetti, or glitter is allowed in the mansion or on the grounds.
 - No candles (except for birthday cake candles) or other open flame decorations are allowed inside or outside of the mansion.  
 - Tape decks, CD players, and any audio-visual equipment may be brought and used for your party.
 - NO ALCOHOL IS ALLOWED on the premise during children’s birthday parties.

Rental of the Lake Mansion is at the discretion of the VSA Nevada staff and board of directors. As a user of the Lake Mansion building and/or grounds on the date and time specified below, I/we assume responsibility for any injury that may occur before (set-up), during or after (clean-up) the event by one of our guests, staff, volunteers or the public. We are responsible for any damage to the building or grounds that may occur before (set-up), during or after (clean-up) of the event. 

Payment due at time of reservation of $200 for up to 12 children. If expecting more than 12 children, add $10 for each additional child.

CANCELLATION POLICY
 - Cancellations at least 7 days prior to event - $150 refund, $50 fee.
 - Cancellations 7-1 days prior to event - $100 refund, $100 fee.
 - Cancellations on day of event or no-shows - no refund, $200 fee.
